About the role
The Occupational Therapist (OT) is a part of the Housing Relocation Support Program who is supporting public housing residents being impacted by housing redevelopment in North-West Melbourne.

The OT role is part of a multidisciplinary team providing a range of supports to assist residents transition into a new location. The OT role will identify and respond to environmental and social barriers which may impact vulnerable residents.

You'll work closely with people from culturally diverse backgrounds, those with living disabilities, chronic or mental health conditions, or older adults, to create personalised and meaningful care plans. Trauma informed and culturally safe practice is central to this position.

Your duties include:

  • Support the ongoing review and improvement of the Housing Relocation Support Program to be responsive to the changing needs of residents
  • Intake as specific assessments, home safety management and complex care case management
  • Participate in multidisciplinary coordinated care activities to provide wholistic care

Our ideal candidate:

  • Tertiary qualification in Occupational Therapy
  • Registered with Australian Health Practitioner Regulation Agency (AHPRA) or intend to register
  • Excellence in the delivery of community based Occupational Therapy services and demonstrated ability to work effectively across a multidisciplinary service and with marginalised clients
  • Strong belief and personal alignment in cohealth Values and Mission
