About our program
Our Community Asthma Program (CAP) is a paediatric asthma program funded by the Department of Health. Offering services to children and young people with asthma aged between 0-18 living in the northern and western suburbs of Melbourne. The key purpose of the program is to improve health outcomes and increase client confidence and to support self management of their health condition. This includes using and navigating health services appropriately and enacting written asthma action plans. This is achieved by providing free education and support to young people, their families and wider circle of care to assist them to better understand asthma.
The aim of this position is to improve health outcomes for our clients by offering direct support and provide families with coordinated links to appropriate health professionals and community agencies. Within this role, you will work individually with referred clients and operate within a coordinated team of educators, primarily across Melbourne's inner northern catchment area and other CAP catchment areas as required.
The key skills & expertise required for this role include:
- Registered Nurse with a minimum 5 years' experience in Paediatric Nursing;
- Demonstrated experience working with children, families and people from diverse backgrounds;
- Confidence to critically assess and provide appropriate, effective and timely care in a multidisciplinary approach;
- Understanding of, and experience in, health promotion, program implementation and evaluation using a community development framework; and
- Asthma educators course.
