About the role

In this role, you will support the delivery of the Housing Support for the Aged program, working with older people living in social housing who have multiple and complex needs and are at risk of homelessness.

You will provide short- to medium-term case management support, assisting clients to maintain their housing and independence. This includes connecting clients with appropriate health, welfare, and community services to address unmet needs and improve their overall health and wellbeing.

Your duties include:

  • Provide outreach support and manage an allocated caseload using a short- to medium-term, client-centred case management approach.
  • Conduct comprehensive needs assessments and develop targeted, achievable care plans that address clients' holistic needs.
  • Facilitate and advocate for clients' access to appropriate community resources and services, including health, counselling, employment, education, recreation, housing, and substance use support.
  • Build and maintain effective relationships with service providers, partner agencies, and key community stakeholders to optimise client outcomes

Our ideal candidate:

  • A tertiary qualification in Social Work or a related discipline.
  • Demonstrated ability to engage respectfully and effectively with people from culturally and socially diverse backgrounds, including marginalised and vulnerable groups.
  • Experience in the health or community services sector, with well-developed skills in care coordination and case management, particularly with clients with complex needs.
