About the role

This position sits within the AOD Outreach cluster, which aims to improve the health and wellbeing of people who use alcohol and other drugs.

The Community Health Nurse will use multi-disciplinary, assertive, integrated, and flexible care approaches which addresses immediate health and support needs, coordination and consumer directed care planning, through to longer term support to clients with complex presentations, either individually or in a group.

This position is located in Melbourne CBD, requires to work shifts from 8 am to 4 pm and from 3 pm to 11 pm, and may require to work weekends, and public holidays.

Your duties include:

  • Participate in assertive street outreach
  • Provide high quality, recovery-oriented, evidence-based primary care services to clients
  • Provide direct service delivery to clients with complex presentations, either individually or in group settings

Our ideal candidate:

  • Tertiary qualifications in Nursing and eligibility to join AHPRA
  • Sound knowledge of the health and social issues for people who experience homelessness and use drugs
  • The ability to assess client need and develop a care plan in order to support and assist their navigation of health and other community services

All appointments are made subject to a satisfactory Police Check, a Working With Children Check (WWCC), complete a Pre-Employment Declaration Form, evidence of legal rights to work in Australia, and evidence of vaccination against COVID-19 (3 doses). Additionally, some roles will also require current AHPRA registration and/or NDIS Worker Screening Clearance.

Roles that are determined to be Risk Category A or B [staff that have direct and/or indirect contact with body substances and are at higher risk of occupational exposure to infections] will be required to provide evidence of immunisation assessment including Influenza prior to commencement [A] or within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders for COVID-19 and legislated requirement for influenza vaccination.
