About the role

The role of Paediatric Asthma Nurse sits within the Community Asthma Program (CAP), which aims to improve health outcomes and support clients to achieve self-management of their health condition.

The Paediatric Asthma Nurse provides asthma education and support to young people and their family and carers. The CAP is available to children and young people with asthma aged between 0-18 living in the northern and western suburbs of Melbourne, with the role being based at the Braybrook sites.

Key Responsibility Areas

  • Accept referrals from the acute sector, GP's and other health professionals
  • Conduct initial assessment and education sessions delivered using a clinic based or outreach approach with standard assessment tools and phone follow up
  • Liaise and implement best practice asthma education and management approaches in conjunction with GP, specialists, community health
  • Initiate and generate individual client care plans and goal setting with clients

Position Requirements

  • Registration with Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse, along with experience in paediatric nursing.
  • Completion of the Asthma Educators course.
  • Demonstrated experience in the provision of services to people of culturally and linguistically diverse backgrounds.
  • Understanding of, and experience in, health promotion, program implementation and evaluation using a community development framework.
  • Experience working within a community-based environment particularly within the field of community health.
  • Understanding of, and experience in delivering paediatric asthma education
  • Experience in writing high quality reports and clinical interviewing
