About the role
This Social Work position offers a dynamic opportunity to provide short-term casework, psychosocial assessment, and care planning within an inter-professional team. You will address issues such as housing, financial hardship, NDIS access, and advocacy, while delivering high-quality, client-centered care. The role also involves secondary consultation, capacity-building, and health promotion, all within a trauma-informed, recovery-oriented framework.

Key responsibilities include:

  • Lead the West Mental Health and Wellbeing Social Work service, ensuring ethical, evidence-based, client-centred casework.
  • Provide short-term casework, assessment, and care planning.
  • Address psychosocial issues such as housing, income support, and NDIS/My Aged Care access.
  • Deliver advocacy, secondary consultation, and capacity-building activities within a multidisciplinary team.
  • Engage in reflective practice, supervision, and professional development.

Our ideal candidate:

  • Tertiary qualification in Social Work and eligibility for AASW membership
  • Proven experience in short-term casework, psychosocial assessment, and care planning for clients with complex needs
  • Ability to navigate multidisciplinary settings and provide advocacy and service coordination
  • Experience working with marginalized groups, including CALD, Aboriginal, Torres Strait Islander, and refugee communities
  • Victorian Driver's License
