About the role
This position sits within the Homelessness Cluster which aims to improve the health, wellbeing, housing, and social outcomes of people experiencing homelessness in the Inner North-West of Melbourne.
Peer Workers address immediate crisis support needs, through to longer term support to establish safe and affordable long-term housing.

Your duties include:
- Share lived experience where it is appropriate relevant, and purposeful to the recovery journey of the consumer
- Provide community-based support in partnership with clients, that is strengths-based, recovery-oriented, and goal directed
- Work collaboratively with clients, carers, and community services to conduct assessments, identify goals, and support the implementation and review of care plans
- Facilitate peer-led and therapeutic groups that support recovery, social connection, and skill development
- Develop and maintain strong working relationships with internal and external stakeholders

Our ideal candidate:
- Demonstrated lived experience and/or relevant tertiary qualification with the ability to use personal experience to support others
- Sound understanding of the Social Determinants of Health, Trauma-Informed Care, recovery models, and strengths-based practice
- High-level interpersonal skills, including the ability to form respectful, professional relationships
- Strong belief and personal alignment in cohealth Values and Mission
