4. Hourly rate :
The hourly rate for this position will be between $37-$39 per hour, depending on experience. As per the Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 2013 – 2015, Social Worker Class 2 Year depending on experience (SCHADS level 5). 5. Knowledge:

6. Qualification:
Do you hold a relevant tertiary qualification or equivalent experience in the mental health, general health, housing, disability or welfare sectors? If so, please communicate

7. Experience:
Do you have experience working with clients who have a history of homelessness and multiple and complex needs?

8. Experience:
Do you have experience in undertaking, supporting and coordinating assertive outreach and group activities

9. Knowledge:
Please communicate your understanding of the Social Determinants of Health and Trauma Informed Care Practices. (Suggested word limit 200 words)

10. Experience:
Do you have previous experience establishing respectful professional relationships that have clear boundaries with consumers, staff and partner organisations? If so, please communicate (Suggested word limit 200 words)
