As our expert, you will lead program level strategy, service planning and design, assuring effective implementation and sustainability of all aspects of the service delivery functions. Responsible for service innovation, strengthening performance, generating sustainable business growth, enhancing team work and collaboration, and assuring compliance and effective management of risk across the Cluster. Programs within the Cluster are anticipated to operate as high performing and effective teams. You will be responsible for a budget of approximately $8.4 million. There is substantial formative and service re-design work to be achieved within this Cluster.

Working closely with the Network Director and cohealth's Transformation Hub, to redesign programs and services to achieve optimal impact, as cohealth shapes leading, contemporary, values-based models of healthcare practice. Teams will, over time, move to consistently achieving best possible client and community outcomes, while meeting service targets and financial benchmarks through enhanced team-based and self-management approaches.

Within this role, you will hold accountability for a complex and significant suite of cohealth's programs and services including:

  • Essendon Adult Day Care;
  • Allied Health (Kensington and Niddrie) - Teams will transition to separate CHSP and Community Health funded programs
  • Hospital Admission Risk Program (Melbourne Health and Western Health)
